Chiropractic & Nutritonal Intake
Please arrive 10 minutes early for your session to fill out the necessary intake forms.
If you have out of network Chiropractic insurance coverage we can provide you with a "superbill" or claim form to fill out and submit yourself to your insurance. You will then be reimbursed personally by your insurance company.
All fees are due at the time of service and package prices are due on the first appointment.
Discounts are given when multiple sessions are purchased at once.

Reminder calls are a courtesy and we do our best to inform you of your appointments ahead of time, however, because we frequently have waiting lists all missed appointments without 24 hour notification will be charged as a missed session.
